SMS Opt-In — Example Form

Retention IQ is a B2B platform used by service businesses (medical spas, dental practices, salons, fitness studios) to communicate with their own past clients. Every business that uses Retention IQ collects explicit, written, opt-in consent from each client before any text message is sent through our shared sending number.

Below is the standard consent form template that our customers embed on their own intake or booking pages. The exact business name, brand colors, and contact information are replaced per customer; the consent language, disclosures, and opt-out instructions are identical across all customers.

Context for reviewers: The form below is what end-users (clients of Retention IQ's customers) see and complete. Consent is captured per individual client through this mechanism and stored against their record. Retention IQ never sends to anyone who has not actively checked the consent box.

Multi-customer model

Retention IQ is a multi-tenant SaaS platform. Each customer business (e.g., Grey Aesthetics) maintains their own list of opted-in clients. Per-client consent is captured at the customer's point of intake using the form above (or an equivalent), stored in our database, and surfaced as part of the recipient's record before any send. The platform refuses to dispatch to recipients without an active opt-in record. Per-customer audit trails of consent are available on request.
